Case managers must learn to help physicians care for hospitalized patients by facilitating decision-making and connecting people with information in a timely manner. All of this results in faster care and better hospital throughput.

Case Management is an Efficiency Catalyst.

While the rest of the clinical team is focused on “getting things done,”  effective case managers makes sure that nothing is overlooked, including the detection and correction of incomplete tasks.

But this isn’t the impression that most hospital executives have of case managers. That’s because many case management programs are not well-designed or managed.

Case managers  trained  to organize their time to be able to make a difference in day-to-day patient care. Case managers must learn to help physicians care for hospitalized patients by facilitating decision-making and connecting people with information in a timely manner. All of this results in faster care and better hospital throughput.

Case Management program should deliver:

  1. Better use of observation admission status
  2. Length of stay reduction by shifting to outpatient or home care
  3. Reduced readmissions
  4. More appropriate use of higher acuity beds
  5. Capacity improvements without adding physical facilities
  6. Faster ER patient throughput because more beds will be available in nursing units 
  7. Simplified and more efficient discharge planning
